Faircape Health Tokai Estate is a Sub Acute Care and Rehabilitation Centre that provides restorative care and rehabilitation for patients recovering from a variety of medical conditions that include Neurological, Orthopedic, Surgical, and Respite. We are a rehabilitation facility seeking a dedicated and detail-oriented Half-day Rehabilitation Research Assistant ( 6 month contract) to conduct one-month post-discharge follow-ups with patients. The primary responsibility will be administering the Modified Barthel Index (MBI) to assess patients' functional outcomes, as well as gathering additional information on their recovery, including any falls, hospital readmissions, or complications experienced after discharge and supporting research initiatives aimed at monitoring and improving rehabilitation outcomes.

Key Responsibilities
- Patient Follow-Up:
- Conduct one-month follow-up calls with patients to assess their progress and gather feedback on their rehabilitation journey.
- Document and report on patient outcomes (Modified Barthel Index (MBI), QOL) using standardized script or questionnaire.
- Ask follow-up questions to gather information on:
- Any falls since discharge.
- Any hospital readmissions.
- Any complications or significant challenges experienced during recovery.
- Accurately record MBI scores and responses to follow-up questions.
- Maintain a well-organized database or tracking system for all follow-up activities.
- Assist the researcher with auditing and organizing patient data for accuracy and completeness.
- Collect and document data related to patient feedback and clinical outcomes.
- Identify and document any issues, concerns, or areas requiring further intervention.
- Escalate complex cases or concerns to the clinical team for review.
- Compile monthly summaries of follow-up findings, highlighting trends and patterns in patient outcomes.
- Provide actionable insights to inform rehabilitation programs and services.
- Schedule follow-up calls and send reminders to patients and caregivers.
- Ensure accurate patient contact details are updated and maintained.
- Maintain accurate records of follow-up calls and patient data.
- Support the team in preparing reports or summaries of patient outcomes and trends.
- Support the clinical researcher with administrative tasks, including drafting protocols, submitting ethics applications, and managing research documentation.
Required Skills and Qualifications
- Education:
- Bachelor’s degree in Occupational Therapy (OT), Physiotherapy, Social Sciences, or a related field (required).
- Skills and Experience:
- Experience in rehabilitation or a similar setting (preferred).
- Strong interpersonal and communication skills.
- Basic understanding of patient care and rehabilitation principles.
- Attention to detail and accuracy in data collection.
- Ability to work independently and manage time effectively.
- Flexible and able to adapt to a fast paced environment.
- Proficiency in Google Workspace or data entry tools.
- Additional Requirements:
- Familiarity with the Modified Barthel Index (training will be provided).
- Compassionate and professional demeanor when interacting with patients and caregivers.
